North Texas Area Community Health Centers, Inc. (NTACHC) is looking for a fluent bilingual (Spanish) LCSW to work in our Fort Worth clinic, in the delivery of behavioral care services and support.  The right candidate will be team oriented, self-motivated and committed to acting as an advocate for NTACHC’s patient population, work with providers and address behavioral health issues faced in their daily life.

Major responsibilities:

  • Consults with patient and primary care provider about behavioral treatment options; coordinate treatment plans, identify barriers and development of solutions.
  • Monitor patients closely to administer follow up assessments, facilitate engagement in treatment, and complete relapse preventions.
  • Provides brief, focused counseling and intervention for patients who need services.
  • Assist in behavioral changes in patients with, or at risk for, physical disorders to make healthier lifestyle choices.
  • Document the patient’s progress and diagnostic information in the electronic health record (EHR).

Minimum of 1 year of clinical experience in a relevant setting.

Behavioral Health Clinical Care Manager - Irving, TX


Purpose:
Handle inbound and outbound member and provider calls on a clinical phone queue. Ensure members receive behavioral health services appropriate to clinical need. Triage crisis calls to assist with stabilization and access to emergency evaluation. Coordinate services with other departments, such as medical case/care management. Perform utilization management functions, including authorization determinations and care shaping.
 
Education/Experience:
Master’s degree in behavioral health and 2+ years of behavioral health experience. Prior managed care experience preferred. Prior experience with multiple system applications and electronic data entry, including call center and/or health care databases preferred.

License/Certification:
Unrestricted license as Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), Licensed Professional Clinical Counselor (LPCC), Licensed Marriage and Family Therapist (LMFT) or Licensed Psychologist (PhD).
 
Work Environment:
Office-based in Call Center
 
Multiple Shifts:

  • 1st Shift: 930 am - 600 pm
  • 2nd Shift*: 100 pm - 930 pm, includes weekends and holidays*
